Choose the right questions

Ask for the information your team needs to start work, but keep the form short. Long forms create friction and usually lead to worse answers.

Good form fields include order number, account email, issue type, urgency, and a short description of what happened.

Publish the form

After creating a form, add it to your website, help center, or customer emails.
